The Medical Social Services at SingHealth Polyclinics (SHP) plays a critical role in optimising care by working with the medical team and
collaborating with community partners to help patients gain access to appropriate financial and social support. We help patients to
improve their medical, emotional, psychological and social conditions.

Our Services

	We provide medical fees assistance to Singapore Citizens and Permanent Residents, who have difficulty with their medical bills in SHP.
	​We journey with patients and families through difficult times and crises. We support them in implementing the agreed strategies so that they are able to cope with the psychological and emotional challenges during difficult times.
	​We provide risk assessment and formulate safety plans with patients experiencing family violence, abuse, neglect and contemplating suicide.
	We work with other healthcare professionals in SHP to facilitate the application of Intermediate Long-Term Care (ILTC) Services.
	We collaborate with external partners, both healthcare and social care providers to link patients to the relevant services and resources in the community.
