Our Medical Social Worker team champions patients' physical and mental wellbeing through direct support and advocacy. We integrate healthcare with social services, addressing both immediate needs and underlying social factors to help patients achieve best health outcomes.

Our services provided include:

We provide psychosocial assessment which is a systematic evaluation of patient’s psychological and social functioning through gathering information, identifying areas where support is needed and working with stakeholders on appropriate intervention.

We journey with patients and their families through difficult times and crises by working with them to implement strategies that strengthen their psychological and emotional coping.

We assist to apply for appropriate medical fees assistance scheme, ensuring that patients can access to quality healthcare services and enhance their health.

We provide risk assessment and formulate safety plans with patients experiencing family violence, abuse, neglect and contemplating suicide.

We work with stakeholders to facilitate the application of Intermediate and Long-Term Care (ILTC) services for patients, ensuring seamless transitions of care.

We assist to patients and families to navigate the complex systems, empowering them to access to the patients to the relevant services and resources in the community.

We advocate and facilitate the Advance Care Planning (ACP) conversation with patients, allowing them to make decision about their future healthcare arrangement and preferences discussions, helping patients plan for future health and personal care preferences.
