These are the Healthier SG benefits and subsidies accessible at your enrolled Healthier SG clinic:

i. Health Plan follow-up

Start your health journey with a fully-subsidised Health Plan discussion at your first visit to your enrolled Healthier SG clinic, and ensure good progress with free annual check-ins, either by phone or during your other medical appointments*.

*Prevailing fees for services outside Healthier SG scope will apply.

What is a Health Plan?

During your first Health Plan discussion, your doctor will create your Health Plan after getting to know you and your health condition.

A Health Plan is a simple guide that captures your Health Plan discussion with your doctor. It includes steps you can take to stay healthy or manage your chronic condition(s), and Healthier SG screening tests and vaccinations that are recommended for you.

After your first Health Plan discussion, your doctor will review your Health Plan at annual check-ins. An annual Health Plan check-in is a follow-up review that should occur within the next calendar year after your first Health Plan discussion. This could be carried out via a phone call, or when you next visit your doctor for other reasons. During the check-in, you and your doctor will discuss and update your existing Health Plan.

*If you have regular appointments with SingHealth Polyclinics for your chronic condition(s), you do not need to book a separate Health Plan discussion, as the Health Plan will be discussed as part of your regular appointments.

ii. Screening tests and vaccinations

Enjoy special subsidies for Healthier SG screening tests and vaccinations such as influenza vaccines, based on your age, health conditions and vaccination history, for Singapore Citizens.

Early detection and treatment can help prevent or delay the onset of common chronic conditions such as diabetes, high blood pressure and cholesterol, and cancers such as colorectal, breast, and cervical.

iii. Out-of-pocket payments

Fully pay with MediSave for treatment of chronic conditions under the Chronic Disease Management Programme (CDMP) without cash co-payment, up to the MediSave withdrawal limit.

Assignment to a My Care Team

Each My Care Team (MCT) is made up of a multi-disciplinary team of Doctors, Nurses and Health Pals. The MCTs are a part of the clinic and aim to actively engage and empower you to join us in your health journey.

After enrolment to the SHP clinic of your choice, you will be assigned to and be seen by members of the same MCT when you visit the clinic for your regular clinic appointments. With this, we hope you will build rapport and form a long-term relationship with your healthcare team.
