The Primary Tech-Enhanced Care (PTEC) Home Blood Pressure Monitoring Programme supports patients with hypertension in managing their condition effectively from home. It is especially beneficial for individuals with busy lifestyles or reduced mobility who find frequent clinic visits challenging. As an extension of our telemedicine services, the programme enhances care quality by empowering patients through education and self-management, enabling better control of blood pressure, prevention of disease progression, and reduction of complications.

  1. Measure your blood pressure at home using a blood pressure monitor.
  2. Enter your readings in the mobile app, or sync them automatically if you are using a compatible Bluetooth-enabled blood pressure monitor. Your care team reviews the readings and follows up with you if anything looks unusual.
  3. View your readings in simple charts that show how your blood pressure changes over time.
  4. Receive gentle reminders and helpful tips to manage your blood pressure.
  5. Teleconsultations and support are available when you need them.

Eligibility criteria:
• Aged 21 to 80 years old
• Not pregnant
• Diagnosis of hypertension
• No history of atrial fibrillation, kidney failure or heart failure
