Safe Re-opening

Following MOH's guidelines for Phase 1- Safe Re-opening, SHP will resume services in a controlled and gradual manner to minimise crowding.

Deployment of SafeEntry

With effect from 12 May 2020, SafeEntry has been deployed at SingHealth Polyclinics to log the entry and exit of patients, visitors and staff.

SafeEntry is a national digital check-in system that logs the names, NRIC/FINs and mobile numbers of individuals. It helps to facilitate contact tracing efforts.

Declaration of Symptoms & Travel History 

All patients must provide truthful declaration to our staff if you:

  • Have any symptoms including fever, cough, runny nose or sore throat;
  • Have any recent travel history within the last 14 days; or
  • Had come into contact with a confirmed case of COVID-19 infection. 

It is an offence under the Infectious Diseases Act to provide false declaration. 

All are required to submit your Declaration Form before entry. (Note: 1 accompanying person per patient only)

For the safety of everyone, we would like to advise all patients to put on masks when you come to the polyclinic. Please approach our staff if you need assistance. Thank you.
