Video Consultation for Chronic Medical Conditions (click here) (ONLY for Existing Patients Already on Follow Up with SingHealth Polyclinics)
A chronic medical condition is a health condition that requires long-term medical follow-up. Examples include: - High Blood Pressure
- High Cholesterol
- Diabetes
- Hepatitis B Carrier
- Thyroid Disease
- Gout
Home blood pressure machine is required for the video consultation if you have high blood pressure, high cholesterol, diabetes, kidney disease, heart disease or history of stroke.

Family Planning (click here) (ONLY for Existing Patients Already on Follow Up with SingHealth Polyclinics)
- For women who are existing patients of the SHP Family Planning Clinic and requires oral contraceptive pills
- Self-measured blood pressure, weight and height measurements are recommended for the consultation

Limitations
Our Video Consultation Services are NOT for urgent or emergency care. Should you require urgent medical care, please seek attention at the nearest medical facility or emergency department or call 995.
Emergency symptoms include but are not limited to:
- Chest pain
- Breathing difficulties
- Severe abdominal pain
- Sudden onset of face droop, limb weakness or difficulty in speech
- Sudden loss of vision
- Fits or seizures
- Severe injury or uncontrolled bleeding
- Severe allergic reaction
- Drug overdose
For conditions that cannot be safely assessed or treated without a physical consultation, the doctor may advise you to seek face-to-face medical attention for further assessment.
