What is Healthier SG (HSG)?
Healthier SG is a national initiative by the Ministry of Health (MOH) that aims to help all Singaporeans take steps towards better health and quality of life in the years to come. Preventive care is key to living long and well. Healthier SG empowers you to stay healthy, detect and manage health issues early for a healthier you.
One Patient, One Care Team
In support of Healthier SG, My Care Team will work with patients to develop a personalised health plan, to take charge of one’s health by going for regular health screenings and staying up-to-date with vaccinations.
Patients will be encouraged to visit their chosen primary care provider regularly for all health care needs and to have better control over any health conditions.
Who is eligible?
You can enrol if you are:
- 40 years old and above
- A Singapore citizen or permanent resident (PR)
SMS invitations will be progressively sent to eligible residents to enrol with a Healthier SG clinic or polyclinic of their choice via the HealthHub app
