Dr Soo Wern Fern

Dr Soo Wern Fern is a Senior Consultant with SingHealth Polyclinic. She is a Family Physician based at Bukit Merah Polyclinic. She manages both adult and paediatric patients with chronic and acute medical conditions in the community.

She has an interest in medical informatics and is the Chairperson of SingHealth Polyclinic Electronic Medical Records (EMR) Committee. She is Deputy Director of SHP’s Medical Informatics Unit and Deputy Chief Medical Informatics Officer. She is also concurrently the Chief Medical Informatics Officer of SingHealth Office of Regional Health.

She is a Physician Faculty Member of Family Medicine Residency Program and Adjunct Assistant Professor with SingHealth Duke-NUS Family Medicine Academic Clinical Programme (FM ACP).
