SHP engages and collaborates with partners in the community so as to achieve greater integration at the primary care level. This is to ensure continuity of care for patients in the community.
SHP Care Management Program
The SHP Care Management Programme is an initiative where our clinic team partner with community providers to provide care for patients with complex chronic conditions and psycho-social issues. On a regular basis, community providers will report useful information that may impact care of the patient to the clinic care team. The social care providers work closely with our eight polyclinics to deliver care to this group of vulnerable patients.
SHP proactively screens chronic patients for psycho-social issues that may affect how well they can manage their condition.
