SHP Home Recovery Programme Patient Information

For COVID-19 positive patients and their household members

Home Recovery Programme (HRP) is a telemedicine service that provides acute care and support for COVID-19 positive patients and their household members.

If you are COVID-19 PCR swab positive, please visit https://www.covid.gov.sg . MOH will be in contact with you. They will assess if you are suitable for the Home Recovery Programme and assign you a Telemedicine Provider.

Call 995 if you have any emergency symptoms of:

  • Chest pain or tightness
  • Palpitations
  • Difficulty breathing or shortness of breath
  • Wheeze
  • Giddiness or fainting
  • Drowsiness or confusion
  • Severe pain (severity score 8 out of 10 and above)

If you are feeling unwell and have any of the following signs, please call 995 and standby your home monitoring equipment as follows.

FAQs (Home Recovery Programme)

Please continue to self-isolate, rest and recover. We hope you will have a speedy recovery.

You may contact your MOH Home Recovery Buddy at 6874 4939 if you require a care pack to self-monitor yourself. 

After you have completed your 10-day (if vaccinated) or 14-day (if unvaccinated) programme, you will be discharged.

The day of notification of your COVID-19 Positive Swab Result is counted as Day 1.

If you are on HRP and received an electronic Isolation Order, no discharge memo is needed. 

You are discharged after Day 10 (if vaccinated) or Day 14 (if unvaccinated), if you are feeling well. There is also no need for further PCR tests as vaccinated individuals are extremely unlikely to be infectious by this time. 

After discharge, please minimize social contact for 7 days, to be safe.


FAQs (Household Members and Isolation Order)

Individuals living in the same household as a COVID positive case should ensure their COVID positive household member who has received an Isolation order (IO) has nominated them for a HRW through the link in their Isolation Order SMS.

Under the Isolation Order issued to COVID-19 patients under the Infectious Diseases Act, patients are required to adhere to the stipulated movement restrictions and remain in the place of isolation during the entire Home Recovery period. It is an offence to breach the conditions of the Isolation Order. First time offenders may be fined up to $10,000 or given 6 months’ imprisonment, or both. No visitors should be allowed in the place of isolation (i.e. home or specific room). Patient must not leave the home/room and must report and undergo medical examinations or treatment as directed.

 

 

FAQs (Swab Test)

You do not need to take ART tests when you are on Home Recovery.
