What is SingHealth Hackathon 2019?
The SingHealth Hackathon 2019 is a time and space where the SingHealth and Duke-NUS community comes together to brainstorm, ideate and design solutions to pressing medical issues.
Over the course of two days, participants will form teams, exchange insights, hear from experts (mentors) and work with prototyping tools to create solutions that strengthen patient care.
The teams will then pitch their ideas to a panel of judges for support to bring forward their proposal throughout SingHealth.

The theme for the SingHealth Hackathon 2019 is “Care for the Community”.
Singaporeans are living longer, healthier lives and we want to partner them to keep well, get well and age well in their communities and homes. How best can we empower individuals to take charge of their health as well as enhance the quality and accessibility of care in the community?

Challenges to be tackled:
The SingHealth Hackathon will focus on the following challenges:
- Keep Well: How might we reach and deliver better care to more individuals in in need, in the communities that SingHealth serves?
SingHealth is piloting community-based programmes, such as geographically-based Regional Health Systems (RHS)-led Community Nursing teams and the community-based Pharmaceutical Care Services. These care providers face the challenge of caring for local residents with a wide range of health conditions and psycho-social needs. With assignments outside the hospitals, they need to plan ahead whilst continually adapting to those schedules to changing demands on their time.
- How might we improve the efficiency and effectiveness of healthcare professionals operating in the community in areas such as improving communication and coordination with community partners, allied health professionals and other stakeholders?
- How might we enable care providers in the community to reach and engage with more vulnerable individuals, including seniors who are isolated and technological disconnected, as well as to keep their caregivers in the loop?
- Get Well: How might we improve continuity of care after discharge?
Continuity of care ensures that post-discharge patients are on the path of recovery and aims to reduce re-admittance to hospitals. We want to empower individuals to take charge of their health through the use of tools to provide reminders, health insights and action plans for the patients. For example, patient is able to activate digital discharge plans on a personal device that reminds them to take and record their blood pressure and suggest clinic appointments in the case of abnormalities.
Supported by a multidisciplinary team, general practitioners (GPs) and polyclinics provide continuing care to improve patients' health conditions. Being geographically closer, they also offer greater convenience for patients.
- How might we encourage patients with stable chronic conditions that require regular follow-ups, to visit GP clinics and polyclinics (rather than hospitals)?
- How might we design tools or processes to facilitate information flow between the patient and the care team, and to encourage the patient to self-manage his/her condition?
- Age Well: How might we increase awareness and facilitate all Singaporeans to do Advance Care Planning (ACP)?
Advance Care Planning is a series of conversations with patients and loved ones designed to prepare them to make the most informed decisions about their preferences for care and treatment, in light of their life values, beliefs and goals of care. Advance Care Planning is a means to exercise choice and ensures that the patient does not receive treatment that is against his/her desire or choice, whilst reducing the stress on the patient's loved ones. ACP is often dismissed and not prioritised as the topics of ACP could be counterculture and uncomfortable to confront.
- How might we ease the process of the patient and his / her caregiver(s) reflecting on the patient's care preferences and treatment goals if their condition deteriorates?
- How might we enable healthcare professionals engage patients and caregivers to do ACP at various stages of their lives?
FAQs
1. Do I need to be a tech or data specialist to participate?
No. We welcome people with different skillsets and value the contribution everyone brings.
2. This is the first time I’m participating in a hackathon. What can I expect?
Hackathons are meant to be fun! On the morning of day one, participants with project ideas will get a chance to do a one-minute pitch to the audience to attract others to join their team. Mentors and resource persons are invited to offer advice and help the teams develop their ideas.
