RESPECT
We aim to provide care that is respectful towards your personal values, physical and mental status, religion, language preferences and cultural practices.
CONFIDENTIALITY & PRIVACY
Your privacy is important to us and your health information will be kept confidential in compliance with applicable law, including the Singapore Personal Data Protection Act (No. 26 of 2012) (the "PDPA").
We will need to access your medical records, including National Electronic Health Record (NEHR), to provide care during consultation and to check the outcomes of your care and referrals made to other healthcare providers and institutions. Your health information will be accessed only by members in the healthcare team involved in your care and staff supporting internal processes. We may need to provide your health information to the authorities when required by the law.
Where relevant, your photographs, videos and recordings may also be collected by us for the purposes of clinical care, quality assurance and education.

TREATMENT
As your health partner, we will explain your health condition(s) and treatment needs in a way that you can understand. We will respect your decision to accept or refuse treatment. If you refuse a recommended treatment, you will be informed of the potential risks, consequences and alternative plans. If you accept a recommended treatment, you have the responsibility to follow through and adhere to the plan of care.
Where required, we will refer you to other healthcare providers and institutions for further evaluation and treatment of your health condition(s).
RESEARCH
Medical research helps to advance medical knowledge. If you are eligible for any medical research, information relating to the research will be presented to you in a language that you understand.
We will seek your consent and agreement for participation. Your decision not to participate or to opt out at any point of time will not affect the level of care you are entitled to.
FEEDBACK
SHP is committed to improving our services and your feedback will help us to provide better care.

We do not tolerate abusive behavior and action will be taken against those who verbally or physically abuse our staff. We aim to address your feedback and we hope that you give our staff a safe environment to care for you.
PARTICIPATION IN CARE PROCESS
You may decide which family members and friends participate in the care process by making decisions about care, asking questions about care and even refusing care. If you are unsure if you can make decisions regarding your care, please let our staff know.
We will inform you about the process of informed consent, when informed consent is required, and when individuals other than yourself can grant informed consent. Our consent process will be consistent with applicable laws and regulations.
YOUR PARTNERSHIP IS IMPORTANT
We regard you and your family as our partners in care and we ask that you and your family:
- Keep to the same care team
- Provide complete, accurate and updated information about your health and personal particulars.
- Participate actively in planning your care and treatment.
- Adhere to the care plan that was discussed and agreed upon with your primary care team.
- Care for your health and adopt a healthy lifestyle.
- Engage in self-care and self-monitoring of your health conditions (e.g. monitoring your blood pressure)
- Treat our staff and other patients with respect and courtesy.
- Share with us if you have any concerns regarding your care.
- Be responsible in meeting the required fees and charges for the medical services provided to you. Please approach us if you require financial assistance, we are more than happy to help.
- Observe the clinic's operational regulations (e.g. registration and operating hours).
- Use the clinic premises and facilities with due care and responsibility.
