Partners in Community Care Network

The Partners in Community Care Network, or PCC Network, was established in February 2008 by SingHealth Polyclinics (SHP) to function as a coordinating body with the clusters of general practitioner clinics and dental practitioner clinics located near the nine SingHealth polyclinics.

SHP aims to engage private practitioners in the community, to explore collaborations and to share clinical and administrative resources, so to achieve greater integration at the primary healthcare level.

The Partners in Community Care Network provides a collaborative framework that can facilitate integration of the polyclinics and private clinics, thereby providing holistic care to patients at the primary care level.

Community Health Assist Scheme (CHAS)

The Community Health Assist Scheme (CHAS), formerly known as the Primary Care Partnership Scheme (PCPS), was started in 2000 by the Ministry of Health (MOH).

With the enhancement on 15th January 2012, CHAS aims to provide accessible and affordable primary care services to more Singaporeans. CHAS covers selected dental treatment, acute conditions* and ten chronic diseases covered under the Chronic Disease Management Programme (CDMP)^.

Patients who qualify for CHAS will receive subsidies when they visit a CHAS-accredited clinic for their healthcare needs#. CHAS complements CDMP which allows patients to use their Medisave for outpatient treatment of the chronic conditions.

With CHAS and other initiatives to come, we are another step closer to fulfilling our nation’s vision of “One Family Physician for every Singaporean”.

* Coverage for acute conditions is only for patients on the CHAS Subsidy Blue Tier.

^ Chronic Disease Management Programme (CDMP). The programme covers: diabetes, hypertension, lipid disorders, stroke, asthma, chronic obstructive pulmonary disease (COPD), schizophrenia and major depression. In Nov 11, it was extended to also include dementia and bipolar disorder. 

# Healthcare needs are defined as basic dental treatments and acute conditions covered under CHAS and the ten chronic diseases covered under the Chronic Disease Management Programme (CDMP).
