糖尿病远程护理年内全面推广 SHP expands its telecare service
SHP has rolled out its TeleCare service (TLC) at all nine polyclinics to provide better care for patients with chronic conditions. The report highlighted that the TLC which was piloted at SHP-Pasir Ris in Jun 2011 for patients who have poorly controlled diabetes and are on insulin. The TLC nurses would assist patients and adjust their insulin doses remotely, based on their home sugar monitoring, by calling them at two-weekly intervals.
